The divorce you are requesting must have occurred in Florida.

After the divorce has been recorded, the Clerk of Court sends a Report of Dissolution of Marriage to Florida Dept. of Health, Bureau of Vital Statistics. If the divorce was recently granted, it may take up to sixty (60) days to be received by this office from the Clerk of Court. The divorce decree and other court papers remain on file at the Clerk’s office. If the current dissolution of marriage is less than sixty (60) days from the date of this application and you need evidence of the divorce for legal purposes, you may wish to contact the county where the Dissolution of Marriage was granted.

The reports dating from June 6, 1927 to the present. Any divorces prior to June 6, 1927 are obtainable from the Clerk of Court in the County where the divorce was granted. If you need the judgment, contact the Clerk of Court in the county where the divorce was granted.

To order a divorce record by mail, provide a signed letter or application which includes the following information:

• Provide full names of both spouses, including any maiden names if applicable.
• Date of the divorce. Provide the month,day and year. If all dates are unknown, provide at least the year of the event.
• City or county where divorce was granted
• The names of both spouses are required for indexing purposes..
